I give my full consent to the application of my tattoo.*

I acknowledge that tattooing is a permanent change to my appearance and that it has been made clear to me that there is no ability to later change, alter, or remove my tattoo.*

I am the person on the legal ID presented as proof that I am at least 18 years of age.*

I am not pregnant or nursing.*

I will not be under the influence of alcohol or drugs and the obtaining of the tattoo is my choice.*

I understand that there is a possibility of an allergic reaction and infection.*

I agree to follow all instructions concerning the care of my tattoo and that any touch ups needed due to my own negligence will be done at my own expense.*

I agree to immediately notify the artist in the event that I feel lightheaded dizzy or faint before during or after the procedure.*

I acknowledge that all tattoos done on hands, fingers, feet, and necks are not guaranteed and if touch ups are needed I will be charged a fee.*

I have been fully informed of the risks of tattooing including but not limited to infection, scarring, difficulties in detecting melanoma, and allergic reactions to tattoo pigment, and nitrile gloves. Having been informed of the potential risks associated with getting tattooed, I still wish to proceed with the tattoo application. I assume any and all risks that may arise from tattooing and I release Stairs Tattoo, Curtis Stairs, Caitlin Stairs, and all its staff of any and all liability.*

I agree to let my artist know if I weigh 350lbs+ so that they can provide alternate seating options*

I understand that all deposits are non refundable and that I must give 48 hours notice if I need to reschedule or I am at risk of forfeiting my deposit.*

I understand that the shop is CASH ONLY and $200 per hour*
